FCAI is offering members access to a group health insurance plan that is based on the participation of multiple FCAI member companies and their non-union employees and eligible dependents. We are working with Daniel Crantz at The Benefits Source to secure a plan.

Program Overview
The FCAI Health Insurance Program provides FCAI member contractors the opportunity to obtain group health insurance quotes for eligible non-union employees and their families. Coverage and pricing are based on pooled participation across participating FCAI member companies.
Requesting a quote does not obligate a company to enroll.
Who Is Eligible
The FCAI Health Insurance Program is available to:
- FCAI member contractors with a valid Federal Employer Identification Number (FEIN)
- Non-union employees of participating member companies
- Employees’ spouses and dependents, if coverage is requested
Additional eligibility considerations:
- Companies must have a minimum of 2 non-union employees to request a quote
- Companies with 3 or more employees may receive more competitive rate offers
Eligibility, coverage options, and pricing may vary based on company participation, employee census information, and insurer requirements.
What Information Is Needed
To prepare an accurate quote, basic information is required for each employee and any family members who would be included on the plan.
For each employee:
- Name
- Date of birth
- Gender
- Zip code
For each family member to be covered (spouse and/or dependents):
- Name
- Relationship to the employee
- Date of birth
- Gender
- ZIP code
This information is used solely for quoting purposes.
How to Provide the Information
Employee and family information is typically collected using a census spreadsheet, but use of the spreadsheet is not required.
You may provide the required information in any of the following formats:
- The census spreadsheet
- A Word document
- An email with the information clearly listed
The important thing is that all required information is included — not the format you use. Send the information to Daniel Crantz, dcrantz@thebenefitsourceinc.com.
How to Request a Quote
- Gather the required employee and family information
- Submit the information using the census spreadsheet, Word document, or email
- The program administrator will review the information and prepare a quote
- Plan details and pricing will be provided for your review
- Download the census spreadsheet here.
- Send your complete information to Daniel Crantz, dcrantz@thebenefitsourceinc.com.
Important Notes
- Requesting a quote does not require enrollment
- Coverage options and pricing are determined by the insurer
- Final accurate offer will be based upon demographics of final enrolled census.
- FCAI does not administer health insurance plans directly
